  • Emergency/Crisis Communication Procedures

    In the event of an unexpected emergency, early school closure or crisis.
  • Cancellation Before School Begins

    A school closure announcement will be sent by text to students and to parents/guardians by 6:45 am. The announcement will also be posted on our website’s homepage by 6:45 am, the school's telephone greeting by 7:15 am, and if the school is closed, After School Care and all extra-curricular activities scheduled for the day will be cancelled.

     School Closure During The Day

     If the school is already open, it will be kept open unless very unusual circumstances occur. In the event of an emergency school closure during the day:

    •  A school closure announcement will be posted on our website as soon as possible, prior to the early closure.
    • Communication with parents/guardians will occur via telephone and email as indicated by you on the form provided on the reverse side.
    • Direct contact with a parent/guardian is important before sending a child home. If no direct contact is made, the child/children will remain under the supervision of the principal and/or designated staff until contact is made.
    • It is important that we have your accurate contact information at all times. The contact names and telephone numbers provided on student registration forms will be used. If this information changes, it is essential that you notify the school office as soon as possible.

  • Medical Information

  • Immunization information 

    All children attending The Linden School between ages 4 to 17 need to be immunized according to Ontario’s Immunization Schedule. Under the Immunization of School Pupils Act, children need to have proof of immunization against diphtheria, tetanus, polio, measles, mumps, rubella, meningococcal disease, whooping cough, and – for children born in 2010 or later – chickenpox to attend school. Children who are not fully immunized may not be allowed to attend school.

    The Linden School requires all students to provide immunization documentation or confirmation of exemption to the school. The principal can defer entry to the classroom/school if this is not submitted.

  • The Linden School is a NUT-FREE environment!

    Knowing that there are students who have severe food allergies, sometimes severe, we request NO nuts or nut products be included in student lunches or snacks.

    • It is the responsibility of each family to inform the school on this medical information form about allergies.
    • This information will be communicated to the students teachers and will a accompany them on any field trips.
    • We ask that students are made aware of ingredients which may be harmful to them.
    • We request your help in ensuring that The Linden School remains a scent-free enviornment
  • In the event of an emergency, the school is authorized to request emergency treatment. 

    I hereby consent to the disclosure of health information, which will enable the school personnel to meet my child's needs.

  • In-Class/Short Excursion Field Trip Permission

  • 1. To participate in extracurricular activities and field trips provided such extracurricular activities and field trips are supervised by an adult. 

    2. To travel to and from such extracurricular activities and field trips by any of chartered bus, taxi, TTC or private car driven by a staff member or parent.

    I agree to release and discharge The Linden School, members of it's staff and its agents, from any liability or loss, damage or injury in connection with and consent to any medical treatement by a qualified practioner that may be necessaray should an emergency arrive.

     

  • Student Media Release Consent Form

    Please provide your consent for us to include your daughter in our publications by completing the form below.
  • We celebrate and promote our school community and student achievements by sharing photos, videos, audio recordings, and written articles that include our students in school publications, advertisements, and communications.

    We would like to have your consent for us to include your daughter in our publications by completing the form on the next page. You have the choice of allowing us to publish your daughter’s photo or school work with or without her name. Your daughter’s name will not be used for advertising purposes, including school brochures and signage.

    If you decide against giving your consent please understand that this means if your daughter is in a photo with other students, we will not be able to include that photo in the school newsletter or the website.

    I agree and give permission for my daughter's image, student work and performance to be photographed, filmed, audio-taped or videotaped for the purpose of being displayed on school premises, published and/or posted on Linden's website, newsletter and digital media channels, and used for advertising as determined by THE LINDEN SCHOOL.

  • VOLUNTEERING AT LINDEN

    Volunteers are essential for Linden to be able to offer the programs and activities we do. Volunteering is also a great way to get connected with other community members! Please let us know how you could support Linden by completing the form below. For those parents who completed a volunteering survey in the fall, we would appreciate it if you could complete this form as well - the last form was focused on volunteering while pandemic restrictions are in place, and we are all hoping to be able to return to in-person activities for the 2023-24 school year!
  • Disclosure Requirement

    Providing a quality education with attention to each student’s particular learning needs is important to The Linden School. While we do provide accommodations for students with special learning needs, there are some we cannot manage. To avoid admitting students with needs we cannot meet, we require full disclosure of student learning or behavioural challenges in the admissions process. With the signature below, I/we confirm that we have shared any documentation or information available regarding student learning, behaviour, medical conditions, or anything else pertinent to their ongoing needs as a student.
  • Fee Agreement

  • With the completion of this registration form, I/we agree to pay all school fees, interest and other disbursements incurred by the Registrant or on the Registrant’s behalf in accordance with the Fee Schedule in effect at this time (included), and comply with and ensure that the Registrant complies with all rules and regulations of The Linden School as outlined in the Family Handbook.
